As far as your university affiliations are concerned:
The rule is that you must mention Humboldt-Universität zu Berlin as one of your affiliations if you sit here in Luisenstraße 56 or in other Humboldt-Universität buildings and/or use Humboldt-Universität-/RTG-managed facilities and services (such as during the teaching program, for soft-skill courses, conflict consultation, travel expenses, equipment, research support, etc.), irrespective whether your employment is with Humboldt-Universität or with Charité or with Magdeburg University. I.e., this applies to all of the doctoral candidates!
As a general rule: Please do not invent institutional translations! Humboldt-Universität zu Berlin is always Humboldt-Universität zu Berlin (there is no accepted English version!), and all faculties and institutes have their own official English translations.
